Undo And Clinical Corrections

SINA preserves clinical history. A correction changes the effective chart without silently deleting the original saved entry.

In plain text: both paths preserve history. Immediate Undo is a real persisted operation; a later correction records a reason and may link a replacement.

Immediate Undo​

Use Undo only when it is offered for the item you just saved. Eligibility is limited by the current user, organization, patient, active visit, entry version, and the configured chairside correction window.

  1. Review the saved entry.
  2. Choose Undo.
  3. Confirm the action when requested.
  4. Wait for the server-confirmed chart refresh.

Undo is a persisted operation. It is not a local hide and it does not erase the original audit history.

Historical Correction​

When immediate Undo is unavailable, use the authorized correction or void flow:

  1. Open the saved entry or tooth history.
  2. Choose the correction action available to your role.
  3. Enter a clear human-readable reason.
  4. Add a replacement entry when the clinical record requires one.
  5. Review the effective chart and history.

The original, correction reason, actor, visit context, and replacement link are kept in history. Completed visits reject new findings and procedures, so later changes must use the correction path.

When To Ask For Help​

Stop and ask an authorized clinic lead when the wrong patient or visit was used, the correction action is unavailable, or the saved history does not match what you expected. Do not create a second patient or visit to conceal a mistake.
