Odontogram Charting

The Teeth section in Patient Record is the current primary charting surface. It displays the 32 permanent adult teeth using FDI identifiers and combines the whole-mouth chart with a focused crown-and-root control for the selected tooth.

Select The Clinical Location​

  1. Select a tooth from the whole-mouth chart.
  2. For a surface finding or restoration, select one crown surface in the focused tooth. For root-canal work, select the root. Whole-tooth actions such as missing, crown, extraction, or implant need only the tooth.
  3. Choose one permitted finding or performed procedure.

Posterior and anterior teeth expose only their applicable surface vocabulary. The interface rejects a surface that does not belong to that tooth. FDI numbers remain left-to-right in Arabic so the anatomical arrangement is not mirrored by the page direction.

Draft, Save, And Retry​

Selecting a tooth, location, or action never writes a clinical record. The draft strip shows the action, FDI tooth, and surface for review. Save is enabled only when the selected action has a valid target.

While a request is in progress, duplicate submission is blocked. A failed save keeps the exact action available for retry. The retry reuses its client request identity, so it cannot silently create a second copy. A successful save clears the local action and note, refreshes the chart and patient history, and leaves the clinician in the same patient context.

Optional Note​

The note under the chart belongs only to the next selected chart action. Use Save with note to persist both together. A note without an action cannot be saved, and a successful save clears it so text cannot leak into a later entry.

Status And Undo​

The status area distinguishes an unsaved draft, saving, saved, and failed states. A color or local highlight is never proof of persistence.

When the server marks the latest history entry as undoable, Undo creates the authorized reversal through the current patient and optional Visit context. It does not erase audit history. Older, ineligible, or already reversed entries do not receive a client-side undo shortcut.

Reading The Chart​

The chart combines text, symbols, outlines, fills, and patterns so meaning does not depend on color alone. A missing or extracted tooth is not presented as a healthy tooth. Root, crown, implant, fracture, finding, and procedure marks come from persisted chart state; the drawing does not infer a diagnosis.

The crown-and-root shapes are charting controls, not radiographs, patient-exact anatomy, or treatment advice. Pediatric and mixed dentition are not supported in this release.

Permissions And Safety​

  • Dental read permission is required to load the chart.
  • Findings and performed procedures have separate write permissions. A user with read-only access sees no editing controls.
  • The patient identifier and any Visit identifier are checked by the backend; page visibility is not an authorization boundary.
  • Switching patient, Visit, or initial-tooth context clears the local draft.
  • Opening Account, Appointments, or History creates no clinical or financial record.
